Need to find a provider quickly? Our online provider search gives you flexibility in a simple format. We update the online search weekly. Just click below on the provider search link in the blue box.
Always verify that the provider you choose is participating in our network before you receive services. While it is our goal to provide accurate information, our provider network is subject to change without notice. Providers in all areas of our service region are included in the provider search. Please be sure to select the correct product (PPO, etc.) when beginning the provider search.

This search is for the Coventry Flex, Coventry HMO & Premier, Coventry POS & Premier Plus, CoventryOne and Coventry National Network products. Just choose your plan from the ones listed.
- If you are a Premier Select (PPO) member or member enrolled in the Passport Program, use the Coventry National Network search link.
- If you are a Choice Plan member, you will search under Coventry POS & Premier Plus or under Coventry National Network, depending on your plan type.
In addition to physicians, you will be able to find hospitals, ancillary service providers (including convenience care clinics) and urgent care centers. If you have any questions about providers and their availability to you, please call Customer Service at (800) 395-2545. - If you are a Premier Select (PPO) member or member enrolled in the Passport Program, use the Coventry National Network search link.

Laboratory Services
Network providers are required to use participating providers for all services, including laboratory testing. Quest Diagnostics, Inc. is Coventry Health Care of Georgia’s preferred outpatient diagnostic laboratory services vendor.

Routine Vision Care
Treatment of diseases and injuries of the eye are covered under your medical benefits and are not part of your routine vision care benefits. For diseases and injuries of the eye, please refer to your plan document for information on covered medical services from a Coventry participating ophthalmologist or optometrist. The specialist office visit copayment listed in your Schedule of Benefits will apply to these medical visits.

Assurant Health Plus Individual Plan
Assurant Health Plus Individual Plan members: If your ID card indicates that you have access to the Coventry Health Care network of providers, this online search will assist you in locating a provider near you. This directory lists only the medical providers affiliated with Coventry Health Care Network for your area.
To find a participating provider, click on the link below. On the next page you will need to enter your address and/or zip code information. You must select the “Coventry POS & Premier Plus” product in order to obtain an accurate listing of providers available to Assurant Health Plus Individual Plan members.
